    You will be coordinating and managing a case load of assigned responsibility. Act as a resource to children/youth served, their respective family members or representatives and the family care providers/support staff. Perform a variety of professional activities in support of the Program including but not limited to administration, home visits, contract enforcement, inspections, placements, special projects, and program development.

     

    Responsibilities

    Some of the Responsibilities are:

    • Work closely with involved professionals (occupational therapist, behaviour consultant, dietician, health nurse, etc.) to develop protocols, care plans, and guidelines
    • Assist with annual program outcomes action plans and provides quarterly variance reports
    • Maintain records and documentation on children/youth and care providers/support staff
    • Schedule, attend, facilitate and chair Individual Service Plan meetings. Assist in the implementation and enforcement of the Individual Service Plans. Develop, review & update ISP's
    • Liaise with community members, government organizations, families, officials, professionals, and organization staff to promote community involvement in the program
    • Ensure support staff/contractors complete and submit quarterly reports to the head office in a timely manner. Provide support and guidance to ensure quarterly reports are completed sufficiently and reflect ISP goals
    • Ensure that Individual Program Plans are developed and implemented to assist personnel to carry out programs and activities with individuals served
    • Assist care support staff/contractors to identify and access community resources to meet individuals’ needs such as social, economic, recreational, physical, vocational and educational services.

    Qualifications

    Knowledge, Skills and Abilities

    • Degree in Social Work, Child and Youth Services, Psychology, Nursing and/or equivalent of education, training and experience in related field
    • Two years work-related experience as a case manager (or similar position) with infants, children and youth with disabilities
    • Awareness of and respect of the human rights of persons-in-care
    • Exceptional communication skills; interpersonal, written, and verbal
    • Ability to multitask, prioritize, and meet strict deadlines
    • Valid Emergency First Aid Certificate & CPR Level C Certificate
    • Mandt Certificate (Training provided)
    • Class 5 BC Driver’s License
    • Criminal Record Clearance
    • Reliable Personal Vehicle

    Typical hours of work are Monday to Friday 8:30am to 4:30pm, but flexible depending on operational requirements.

     

    Starting pay rate $26.75 – 31.16 per hour depending on experience
