Assisted Independent Living Case Manager – Burnaby

 

Exciting opportunity exists for the right individual to work in our Burnaby Head Office as an Assisted Independent Living Case Manager. The successful candidate will be joining a progressive non-profit organization with growing opportunities. The Assisted Independent Living (AIL) Program provides accommodations and supports to adults with acquired brain injury, mental health diagnoses, as well as physical, developmental, and medical healthcare needs to provide them with resources to live healthy and fulfilling lives at home and in the community.

 

 Shifts are Monday to Friday 30hrs/wk (Temporary) with possible opportunity for a permanent status.

Responsibilities

Work collaboratively with the department Director and Program Manager to maintain the program in a fun and flexible environment.

 

Coordinate and manage a case load of assigned responsibility. Act as a resource to individuals served, their respective family members or representatives and the care providers. Perform a variety of professional activities in support of the Program including but not limited to overseeing contracted community support homes, connecting with contracted care providers in the community to oversee service provision, coordinating OT, PT and other health care related services to support the individual in service, collaborating with steakholders, reporting to Funders regarding critical planning and incidents, assisting new participants with transitioning into program, overseeing individulaized client centered service plans and ensuring all standrads are upheld. 

Qualifications

 Job Qualifications Include:

  • Minimum Bachelor of social sciences, PT, OT, behavioural interventionist/counsellor, criminology, social work

Job Requirements Include:

-Criminal record check

-TB test

-First Aid and CPR

-Minimum of three (3) contactable references

-Valid BCDL and Reliable vehicle to use for work purposes

 

Additional Requirements:

-Experience in the public or non-profit sector

-Experience working with individuals with physical, developmental disabilities, brain injuries, and/or complex health issues

-Experience working in a team environment

-Experience working with a case management team

-Demonstrated ability to deal with crisis and emergency situations

-Computer literacy and experience with databases

-Excellent written and verbal communication skills

-Excellent time management skills

-Knowledge of community resources

 

 

 *Must be willing to travel throughout Lower Mainland and Fraser Valley  

 *Position includes rotating on-call duties

 

Start Date: ASAP

 

Compensation: starting at $24.78 - $28.26/hr and full benefits upon meeting eligibility requirements after three months.
