Overview

Hybrid Supported Home Share Providers needed 

 

Strive Living Society is a dynamic non-profit organization that strives to provide accommodations and supports to adults with mental health, physical and developmental disabilities as well as challenging behaviors and medical healthcare needs. 


Strive Living Society is currently seeking highly skilled, experienced couples in the Greater Vancouver are to provide a warm and inclusive home environment for individuals with complex medical, social and behavioural needs. Municipalities with the greatest demand for homes are, Vancouver, North Vancouver,  Burnaby, New Westminster, Coquitlam, Port Coquitlam, Port Moody and Richmond. Homes must have a separate suite available for the individual to live in. The Hybrid Supported Home Share program places individuals into caring homes in addition to providing one-to-one staffing 30+ hours per week, as well as intensive ongoing case management support.

 

Contracts for Hybrid Supported Home Share range from $4,000.00 to $7,000.00 per month, depending on the individual’s level of support required. Suitability of the applicant will be determined through a screening process based on an agency interviews, reference checks, submission of all required documentation (including a criminal record background check for all adults over 19 years of age that reside in the home, current first aid certification as well as a medical note).

Responsibilities

 We require care providers to perform duties which may include overseeing medication administration, providing personal care, meal preparation, grocery shopping, assisting with personal finances/budgeting, community integration and transportation to and from medical/dental appointments.

 

Qualifications

 

**Valid BC Driver’s License, clear driver’s abstract and a reliable vehicle 

** Be comfortable working individually and as part of a multi-disciplinary team of professionals

**Valid Home and Vehicle insurance

**Negative TB Test

**NVCI/MANDT training is an asset but not required.

**Extensive experience supporting individuals with complex intellectual, behavioural and/or social needs.

 

All above requirements can be obtained before an individual moves in.  Strive also offers training opportunities for our contractors including a First Aid, CPR and Mandt training.
